If a fail is entered when recording a result, the Add Defect pop-up is automatically displayed.
Enter the following information:

| Description | Enter a description to identify the defect. |
| Notes | Enter additional notes about the defect. |
| Photos (Add Photo) | Tap the Add Photo button to display the camera for the device. Tap on the Take Photo button to take one of more photos of the defect. |
Work Orders with combined steps will automatically select the step relevant to the failure for easier referencing in the Defect List. To change this selection click the drop-down.
Tap on the 
button to save the Failure to the Defect list for the Work Order.
